An individual's name changes (marriage, divorce or legal name change)

Copy of marriage certificate or divorce decree reinstating maiden name or court order evidencing name change
     
Company or corporation changes its name only

Certificate of Name Change (verification of Tax I.D. to be used should be included)
     
Company or corporation's name changes due to merger Certificate of Merger (If Tax I.D.'s are changing, then appropriate recorded conveyances need to be included)
     


PURCHASE OR SALE OF INTEREST

Royalty Interest (RI), Overriding Royalty Interest (O) or Working Interest (WI)   Copy of appropriate conveyance recorded in the county in which the property is located evidencing the acquisition or sale of the property (State and federal interests require filing agency appropriate forms for approvals)
     

TRUSTS / PARTNERSHIPS

Trust or Partnership is created   Copy of Trust Agreement or Partnership Agreement and appropriate recorded conveyance transferring interest to the trust or partnership (verification of Tax I.D. to be used should be included)
     
Change of Trustee (Death)   Recorded instrument evidencing death of Trustee and appointment of Successor Trustee (Copy of Trust Agreement)
     
Change of Trustee (Resignation)   Recorded instrument evidencing Trustee resignation and appointment of Successor Trustee (Copy of Trust Agreement)
